• Abstract

  The IESS Project aims to design and implement an ICT based platform that facilitates the provision of innovative services of primary care and Integrated Home Care (IHC), in particular those kind of services directed almost exclusively to meet the needs of elderly, disabled and patients with chronic diseases.

  

  IESS will try also to improve the mutual communication of information between clinicians and patients and to facilitate the processes of integration between the clinical centres and the health professionals (like General Practitioner), thereby increasing the effectiveness of care for patients. Signo Motus is involved both as project coordinator and as technical partner for the development of the IESS Platform and the implementation/management of the related service at Regional level.

• Main IESS Project Objectives

  To provide an ICT based platform for the delivery of innovative e-health services for the chronic care in order to:

  • Provide continuity of care for vulnerable people through better interaction patientclinicians by expanding the range of services using ICT;
    
    
  • Contribute to improve the management of health and in particular the patient’s quality of life;
    
    
  • Contribute to improve the efficiency of the regional health system according to the principles of appropriateness, quality and productivity.
